If you've been treated for gum disease, standard six-month cleanings aren't enough to keep it from coming back. Periodontal maintenance is a specialized cleaning schedule, typically every three or four months, designed to keep the disease stabilized long-term. For our patients with a history of periodontitis, this is what protects the work we've already done.
Why the shorter interval matters
Periodontal pockets repopulate with bacteria within weeks after a deep cleaning. In healthy mouths, six months is fine because there's no active disease and home care can keep things manageable. In a mouth with a history of periodontitis, the pockets are deeper, the supporting tissue is more fragile, and bacteria have more opportunity to re-establish. Three or four month intervals keep the bacteria at manageable levels before they can cause further damage.
Patients who skip maintenance visits after initial treatment often see their pockets deepen, more bone loss show up on X-rays, and eventually find themselves back in active disease. The schedule isn't arbitrary; it's based on how quickly the tissue-destroying bacteria can recover and reorganize.
What happens at a maintenance visit
Periodontal maintenance is more thorough than a routine cleaning. Your hygienist cleans above and below the gumline, spending extra time in the areas with deeper pockets. Pocket depths are re-measured and compared to previous visits to watch for any changes.
Dr. Osborne examines the site periodically as well, looking for any new areas of concern and ensuring the overall picture is stable. If pockets are deepening or new problems are emerging, we intervene early rather than waiting for the next six-month visit.
Home care between visits
Maintenance patients benefit most from consistent home care. Twice-daily brushing with a soft-bristle toothbrush, daily flossing, and often a water flosser are the foundation. Some patients benefit from antimicrobial rinses or prescription toothpaste. We tailor the recommendations to what your specific mouth needs.
Insurance and cost
Most dental insurance plans cover periodontal maintenance at the standard cleaning frequency (typically twice a year). For the additional visits needed on a three or four month schedule, coverage varies. Many plans cover them fully with documented history; others cover them partially. We verify coverage specific to your plan and give you an accurate out-of-pocket estimate.
